Hospitalization Procedures

Hospitalization Date and Time Information

  1. Our physicians will determine your hospitalization date and time by looking at your condition and the open bed condition. If your hospitalization ward or time changes, a member of staff will inform you about them by telephone.
  2. If you wish to use a private room (fee required) during hospitalization, please consult with us.

Day of Hospitalization Flow and Personal Effects

  1. Please complete the procedures at the Hospitalization Reception on the second floor on the day.
  2. You will need the following things to complete the procedures. Please do not forget to bring them with you.
  • Hospitalization application form
  • Health insurance card
  • Patient's card (ID card)
  • Document given to you when reserving hospitalization (Data base )
  • Inkan (seal)
  • Medical cards (for those who have a elderly payment recipient card, livelihood protection, disabled medical treatment, pediatric medical treatment and worker accident insurance documents)
*You do not need a hospitalization deposit.


Insurance of Medical Certificate and Other Certificates

Please consult with your attending physician if you need a medical certificate while you are hospitalized. If you need a medical certificate after discharge from the hospital, please apply at the Document Reception on the second floor.

Opinion and Consultation Counter

Patient Opinion and Consultation CounterWe handle various opinions, requests and consultations from patients.
Medical Support OfficeOur expert counselors handle the following consultations.
  • Medical expenses (e.g., various public medical expense assistance programs)
  • Various problems in the life during recuperation (e.g., programs to support living, allowances and pensions)
  • Living, nursing care, households, workplaces and schools after discharge from hospital
  • Worries due to illnesses and disabilities
Nursing Consultation OfficerWe handle consultations about life during hospitalization, recuperation at home, and introductions to nursing care supplies and medical equipment.
We provide support to families and consultations over the phone, so please feel free to use this service.


Other Precautions

  • Please understand that we cannot answer inquiries over the phone about patients. This is to ensure the privacy of hospitalized patients.
  • We strive to provide support in foreign languages. However, it may be difficult for us to provide a satisfactorily accurate explanation, so please bring someone who can act as an interpreter as far as possible.
  • We will strongly refuse gifts (e.g., money and valuables) to members of staff.

Life during Hospitalization

This is information on your personal belongings while hospitalized and hospitalization life.

Items You Will Need While Hospitalized

  • Writing instruments
  • (Tea) cup
  • Tissue
  • Toiletries (e.g., soap, shampoo and toothbrush)

  • Underwear
  • Pajamas
  • Dressing gowns or clothes to wear over yourself
  • Sandals / inhouse shoes which are less likely to slip (especially footwear for the elderly)
  • Medicines you currently take and their directions/instructions
*We have chopsticks available. However, you are free to bring your own that you normally use.
*If you will take a surgery, a nurse will give you an explanation on the items you will need after the hospitalization.
*You can buy daily necessities at the shop in the hospital (first floor).
*You can also use a rental sleepwear service (inkan (seal) required).


Diagnosis during Hospitalization

Diagnosis
Your attending physician will explain about your treatment and tests. If you have any questions or requests, please feel free to consult with us. We strive for efficient treatment with the aim of the early discharge of our patients.

Nurses
The nurses responsible for you will be determined by each ward. Other nurses may bear responsibility for you when those nurses have a day off or are not on the night shift. We will provide you a good nursing care for the day of discharge since the hospitalization. Please consult them about anything if something is unclear.

Family Attendants

Nurses will take care of your nursing during hospitalization. This means you do not need an attendant. You may have an attendant if there is a request from a family members and it is deemed necessary by your physician.

PCIf you bring in a PC, we ask that you use it in a private room or a shared ward cafeteria. You are not permitted to use PCs in four-person rooms.

Mobile Phones, PHS and Public Telephones
Please use mobile phones and PHS in the areas where they are available (elevator hall corner on the third to seventh floors). Use in hospital rooms is prohibited. We cannot pass on the telephone to patients, but we can convey the messages.

Smoking

There is no smoking areas in the hospital site.

Lights out Time

Lights out time10 p.m. (in principle)

*Please do not turn on the light or leave your room after the lights out time.


Emergency Evacuation

  • If there is an emergency disaster, please act calmly according to the guidance of members of staff.
  • Please do not use elevators when evacuating to prevent accidents involving becoming trapped.
  • Please check the emergency exits when your are hospitalized.

Hospitalization Rules

  • Please do not bring large amounts of money, valuables and bankbooks. Furthermore, there are simple safes with keys in the bedside tables (television stands), so please use these.
  • We ask that you observe the hospitalization life rules so that you can enjoy your time while hospitalized with peace of mind. We may discharge you from hospital if you inconvenience other patients.

Discharge from Hospital

We are an acute phase hospital. Therefore, we do not offer long-term hospitalization in principle. We cooperate with community medical institutions to give information on hospitals that offer continuing rehabilitation and long-term recuperation or to recommend home recuperation after discharge from hospital.
This is information on the flow of discharge from hospital.
  • Your attending physician will determine your discharge from hospital. Please follow the instructions of your attending physician with regards to your date of discharge from hospital.
  • We will give you a discharge accounting ticket when your are discharged. Please pay this at Accounting Counter 8 on the second floor. Please discharge from hospital after presenting your Discharge Accounting Ticket to the ward after payment.
  • If you need a medical certificate after being discharged from hospital, please ask at Documents Counter 12 on the second floor.
  • We are conducting a discharge questionnaire to become an even better hospital by making reference to comments from our patients. Please cooperate with this.

Hospitalization Expenses

This is information on the payment and calculation method of hospitalization expenses.
  1. The hospitalization expenses are the total amount of medical expenses you must pay, personal expenses pertaining to food and fees for using paid services. This amount differs depending on your type of insurance card.
  2. The hospitalization expenses of patients hospitalized in a general wards are calculated by diagnosis procedure combination (DPC).
  3. You can pay by credit card. You have to pay at once in principle. However, if your amount of payment exceeds 30,000 yen, you can also use installment.

Payment of Hospitalization Expenses

  • We will calculate your hospitalization expenses during your stay in hospital at the end of the month. We will then bill you around the tenth day of the following month. Please pay at Accounting Counter 8 on the second floor.
  • If you are discharged from hospital, we will bill you the hospitalization expenses up to that day when you are discharged. Please understand that we may bill you additionally after you are discharged from hospital due to clerical procedures.
  • We cannot re-issue you the receipt. You will need your receipt when you declare your medical expenses, so please keep it with care.
  • Please inquire at the Hospitalization Reception on the second floor if something is unclear.

Diagnosis Procedure Combination (DPC)

We calculate hospitalization expenses of patients hospitalized in a general ward by diagnosis procedure combination (DPC). DPC is a system to calculate diagnosis and treatment actions (e.g., medicines, tests and X-rays, etc.) as a fixed payment per one day of hospitalization according to your illness (diagnosis classification) instead of volume calculation that accumulates and totals diagnosis and treatment actions.

Clinical Pathways

Clinical pathways summarize the schedule of patients for the treatment and examination of their illnesses. These are made for each treatment and examination. They explain in detail the standard content and schedule of the examinations, surgeries, treatments, post-surgery rehabilitation, foods and baths by day from when a patient is hospitalized. The basic schedule during hospitalization is written in detail. This means that there are advantages such as the patient being able to know the schedule and being able to reduce anxieties about life while hospitalized. Nevertheless, the standard pathway may vary depending on the condition of the patient. In addition, there are also treatments for which pathways have yet to be prepared.
